Athlete Corner

All athletes that would like to participate in a sport must do the following things to be added to a roster.

 

1. Fill out MSHS Participation Form Click Here. Hard copies can be found in the Athletic Office.

 

2. Up-to-date physical MUST be on file with the school or uploaded to Athletic Clearance (form can be downloaded below) BEFORE an athlete can practice/play. 

 

3. Athletic Clearance MUST be complete before participating in any MSHS Sport.

 

Medical History and Physical Evaluation forms are listed below as one PDF and are available for download and use. Physical evaluations are valid for one year from the listed date; all athletes must have a valid physical for the ENTIRE season. If no date is listed, a pre-participation physical evaluation is needed before any participation. All sports can upload their physicals to Athletic Clearance during the registration process. If you do not have access to the internet or a scanner, you can turn in the medical forms to the athletic office.
